With terminal illness, I believe if there is no chance for the patient to get better, it should be the decision of the patient to decide except if the patient does not have the mental capacity to make such a decision. This becomes a very difficult decision, particularly if the patient is in a coma. If there is family, it should be decided with that family. Sometimes people leave instructions about what they would like to happen to them if they are in that state.
