What is a non-psychological phobia?

I always thought that a phobia was an irrational fear of something, so I figured they must all be psychological. Apparently a non-psychological phobia is usually a condition where someone can't handle something because of other medical reasons. For example, photophobia literally means fear of light, but the term might be used to describe someone who can't be exposed to light because of a physical condition. So someone with photophobia might not actually fear light. Has anyone heard of someone having something like that?
